Position Overview:
We are seeking a dedicated and experienced Physician Assistant (PA) to join our patient-centered care team. This role involves delivering primary and behavioral health support across various community and facility-based settings. Ideal candidates must have experience with Managed Care populations and a strong commitment to underserved and geriatric communities.
Key Responsibilities:
- Perform comprehensive patient evaluations and physical exams
- Diagnose and manage acute and chronic medical conditions
- Develop individualized, patient-focused treatment plans
- Prescribe medications in accordance with DEA regulations
- Document all encounters accurately and efficiently in Electronic Health Records (EHR)
- Participate in patient case reviews with the Medical Director
- Collaborate with care teams including behavioral health, nursing, and specialty providers
- Deliver services in assisted living facilities and community-based settings
- Contribute to quality improvement and care coordination programs
- Add language about completing forms from 3rd party insurance companies
Requirements:
- Graduate of an accredited Physician Assistant program
- Current California PA license and NCCPA certification
- DEA license (required)
- Minimum 2 year of experience in Managed Care or similar patient populations
- Strong proficiency in EHR medical documentation
- Reliable, communicative, and organized with strong team collaboration skills
- Availability for regular case reviews with clinical leadership
- Compassion for geriatric and mentally ill patients in underserved settings
- Experience with hierarchical coding systems
- Bilingual (Spanish) is a plus
